The Title “Case Manager” is incorporated to include the registered nurse who assumes the responsibility for coordinating total care for the client. The case Manager must meet all of the qualifications and responsibilities as outlined in the job description for “Registered Nurse”. The Case Manager ensures a holistic approach to the client and the client’s support group. The Case Manager position is of the greatest importance in maintaining quality and continuity in ensuring patient improvement and effective outcome.
Qualifications:*Home Health Experienced Required* •Currently licensure as a registered nurse in the State of Texas. •Graduation from an accredited school or college of professional (registered) nursing. •Minimum of one (1) year of clinical experience as a professional nurse. (Exceptions to this policy can only be authorized by the Administrator and/or Director of Nursing). •Current and valid CPR certification.
Duties and Responsibilities: •Assumes role of total care coordination in respect to the patient. •Reports directly to the clinical supervisor or director appropriate to the patient. •Maintains current/updated patient charts with complete and accurate documentation as required by the State and Medicare. •Established at least monthly communication between all disciplines involved in the care of the client (i.e.: PT, OT, Aide, and MSW). A case conference sheet will be filled out and turned into the office on a regular basis as required by this agency. •Develops a medical update summary to the physician every 10 days. •Assumes responsibility for maintaining lab results and reporting to the physician and director. •Follows progress of patient while in hospital. Coordinates medical care transition from facility to home. •Develops, implements and manages the patient’s Plan of Care. •Continually strives to improve nursing care by broadening and deepening her knowledge through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading. •Supervises and teaches other nursing personnel as needed. •Assumes other duties and responsibilities as assigned by Director of Nursing or Administration. •Corrects all documentation requiring Quality Assurance Action in a timely manner. •Obtains and implements orders for medications and treatments as prescribed by the physician and all aspects of the nursing care supervisor. Updates all medical records as necessary. •Initiates and applies appropriate preventative, therapeutic, and rehabilitative nursing procedures in accordance with physicians’ orders and the nursing care plan. •Assists client with activities of daily living as necessary while promoting the highest level of independence functioning attainable.
Skills •Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
